I understand minor aches and pains are a part of dance - I will listen to my body, mark/modify or walk through movements as needed, but still actively participate in class.

I agree to make my instructors aware of any injury and present a DOCTORS NOTE if I need to sit out, in which case I will attend class to take notes so I do note gt behind

If i sit out of class due to an injury, I agree to sit out of ALL ACTIVITIES that could make the injury worse - INCLUDING DANCE TEAM, CHEER, SPORTS, ETC

DANCE ONLY: ENCORE OR SENIOR (Mon & Wed 6-8pm) I will dance and perform in the company dance numbers (no individual part). Occasionally I may be asked to stay later to work on a specific piece for a performance.

ENCORE DANCE & THEATER: Mon/Wed 6-8 / Mon 8:00-9:30 (AS NEEDED) I wish to audition for a speaking role. I understand I will not be scheduled to rehearse every week, or the full time every week, but some weeks will be needed until 9:30pm

Pointe is a beautiful, but very expensive and painful art form. Students (Generally age 12-14) will be evaluated for pointe at auditions to determine readiness.

I wish to OPT OUT of pointe (even if approved) and continue Ballet in regular ballet shoes.

I wish to continue on Pointe (have already been approved)

I aspire to pursue point WHEN my instructors approves me. I TRUST my instructors to know when I am ready, because they love me and don’t want me to get injured

I will wear my POINTE SHOES every class that pointe is practiced, if I go without shoes more than 3x, I understand i will not perform in them

It is my responsibility to have shoes that are safe and properly sewn & prepared for class each week. If my instructors deems my shoes unsafe, I will remove them and count the day as a missed shoe day.

I understand it is not safe to dance en pointe with an injury. If an Injury prohibits wearing pointe shoes (or requires sitting out / or marking) more than 3x a doctors note clearing the injury will be required before returning to pointe.
